All referrals are made through our centralized Access Center.

(800) 649-3777 and ask them to forward your message to the above number.

When you call the Access Center you will be asked about your mental health problem, how you are now, and if you have been treated for a mental health problem before. You will receive a screening to determine if you meet eligibility criteria. If we are not the appropriate provider, we will refer you to another community service provider.

Bring the following to your assessment:

  • Proof of insurance (including Medicare and Medicaid)
  • Authorization from your insurance company
  • Recent pay stub or tax return if there is no insurance or authorization for treatment
  • List of current medications
  • List of allergies or medical conditions
  • Necessary court documents
    (if you have a court appointed guardian)
