When you call the Access Center you will be asked about your mental health
problem, how you are now, and if you have been treated for a mental health
problem before. You will receive a screening to determine if you meet
eligibility criteria. If we are not the appropriate provider, we will refer you
to another community service provider.
Bring the following to your assessment:
- Proof of insurance (including Medicare and Medicaid)
- Authorization from your insurance company
- Recent pay stub or tax return if there is no insurance or authorization
for treatment
- List of current medications
- List of allergies or medical conditions
- Necessary court documents
(if you have a court appointed guardian)
