All incidents or potential incidents as they relate to consumer health, safety and care must be reported to CMHA-CEI via the CEI Incident Web Portal. An Incident Report shall be completed when staff either witness or are the first to become aware/informed of an incident involving a CMH consumer who is actively receiving services.
